We recognise the challenges faced by young people and their families as they transition from childrens services into adult services, and are committed to ensuring that we have the right conversations with them at the right time to ensure a smooth and seamless pathway.
Moving into Adulthood is therefore a key area of development for Adult Social Care, and this is an exciting time to join us on our journey. You will be pivotal in supporting the continued development of our pathway.
You will be working as part of the Moving into Adulthood Service, and will be aligned to adult social care teams which support adults with Learning Disabilities, Physical Disabilities and Mental Health conditions.
You will work with and have oversight of a range of young people with complex needs, who may also be in specialist childrens provisions, hospital settings or out of county childrens placements. They will largely range from 16 upwards, although you are likely to to be aware of them from an earlier age.
You will be responsible for undertaking timely assessments, support planning and implementation of support under the Care Act 2014, working at the right pace and timescale to allow young people and their families to be fully prepared for the move into adult social care services.
Alongside the great relationships you will develop with a range of partner agencies, including education, childrens and health colleagues, you will also provide advice and guidance to other colleagues within the adult social care teams who are working with young people moving into adulthood. You will also collate information and provide reports to help inform future sufficiency planning.
About Us
West Northamptonshire Council Adult Social Care has been on an exciting journey of transformation as we undertook the change to become a unitary authority in April 2021. This has given us the opportunity to really look at how we provide adult social care on a more localised level, leading to lots of exciting innovations and a new model of working which puts people and their communities firmly at the heart of what we do.
Our Approach
We use a strengths-based outcomes focussed approach and utilise the 3 conversations model. This means engaging people with young people and their families and carers in a conversational model which allows them to tell us what is important and really matters to them, and then taking timely action to support them to connect with resources that help them continue with their chosen life - either with support or independently.
About You
We know that our staff are key to our success, and we are seeking people with ambition, creativity and commitment to our values to help us to achieve the best possible outcomes for young people and their families

- You are a great team player, willing to share knowledge and expertise with colleagues in your team
- You love a challenge, and have an openness to learn and change

- You have a passion for connecting and working in partnership with young people, their families and their situation.
- You are willing and able to work collaboratively with a wide range of partners, including community groups and the voluntary sector
