The care team nurse is responsible for assisting providers using their nursing background to assess, evaluate, and manage the care of all members/patients presenting to the LivingWell clinics. The LPN/LVN will apply nursing knowledge, judgment, and skills to provide patient care. The care that is provided will be in direct correlation to the established Policies and Procedures of HealthSpring and the scope of practice established by the Department of Health.
This role is patient facing & you must be fully vaccinated OR be required to undergo testing twice a week. Note, in some locations local law requires the clinician to be vaccinated with no testing option, in those locations local law takes precedent.
Core Responsibilities/Essential Functions
Responsible for providing a positive patient experience by placing the patient’s needs as the focus
Maintain all patient information in a confidential and secure manner that is in compliance with HIPAA guidelines
Work well with the entire clinic team to provide exceptional patient care
Responsible for providing direct patient care that includes clinical assessment and documentation in the EMR
Knowledgeable about the purpose of the LivingWell clinics and HEDIS/Stars/P4Q measures
Responsible for identification of due measures prior to patient visit
Proactive in obtaining all pertinent medical records needed prior to patient visit
Proficient in lab draw and completion of documentation required for lab processing
Responsible for completion of diagnostic testing in the clinics which include EKG, Spirometry, Heel Scans, Retinal Eye Exams, Tonometer readings, and all vitals
Provide clinical education as indicated for patient specific disease processes
Proficient in EMR documentation that includes all areas of a clinic visit, determined by visit type, and are in compliance with established HealthSpring guidelines
Responsible for coordination of care and communication with referring physicians and facilities
Work with the Health Plan and management to determine community outreach involvement
Initiate follow-up calls to patients with test results, referrals, and medication refills as appropriate
Cultivate healthy relationships with clinic team, referring providers, Health Plan, and enterprise employees

Minimum Qualifications
LPN or LVN with current unrestricted license in applicable state
Current BLS certification
At least two years of experience working in a physician office environment
Demonstrates good verbal and written communication skills
Basic Computer skills

Preferred Requirements
At least three years experience in a physician practice or hospital setting
At least one year working with an Electronic Health Record
Understanding of documentation and coding guidelines
