Location: Remote Based, seeking RN candidates licensed in either the states of Idaho, Utah, Nevada, New Mexico or Arizona. Candidates with active, unrestricted Compact RN license highly preferred.

*Must be Available to Work During Pacific Standard Time Business Hours and/or Mountain Standard Time Business Hours. 

The Remote Care Manager - RN is a licensed clinician who works closely as part of a multidisciplinary team to identify and execute the best plan of care for chronically ill patients. This position will manage a census of patients through both responsive and proactive outreach, identifying acute issues and educating on the progression of chronic illnesses. Will build rapport telephonically with patients while walking through the end stages of the patient’s disease process and helping them identify their goals of care.

Primary duties may include, but are not limited to:

  • Work closely with the multidisciplinary team of Advance Practice Providers, Social Workers and Nursing Support Specialists to manage a census of patients by helping to identify and carry out a plan of care
  • Participate in Interdisciplinary Team Meetings
  • Conduct telephonic assessments of acute and chronic conditions to include acute symptom management, medication reconciliation and Goals of Care Conversations under the direction of the Advance Practice Providers and Physicians
  • Conduct proactive, telephonic outreach to patients with chronic, life-limiting illnesses to help with Disease education, medication reconciliation and Goals of Care Conversations
  • Ensure collaboration with other providers including external physicians and specialists, hospice agencies and Aspire’s multi-disciplinary team
  • Serve as the liaison for the hospice agencies to help ensure smooth and timely hospice transitions

Qualifications

  • Must have a minimum 3 years of clinical experience required, preferably in a centralized care management team environment 
  • Requires current, active, unrestricted RN license to practice as a health professional in the state where you reside
  • Active, unrestricted Compact RN license is highly preferred.
  • Experience working in a team environment that includes a significant amount of collaboration and innovation 
  • Prior healthcare experience serving chronically ill patients 
  • Telephonic case management or care coordination experience preferred
  • Compassionate and professional demeanor required
  • Exceptional organizational skills and ability to multi-task
  • Strong verbal and written communication skills
  • Strong Word, Excel, and database management skills
  • Experience working in a fast-growing, rapidly changing environment
  • Hospice or Palliative Care experience is highly preferred.
  • Hospice and Palliative Nursing certification is a plus.
